What Happens If You Stay Awake for 72 Hours?
Hour by hour, what 72 hours without sleep does to reaction time, hormones, immunity and perception, and why you cannot judge your own impairment.

While you sleep, the spaces between your brain cells open up, and fluid washes out the waste the day left behind. It only happens in sleep.
So the clock does not start when you get tired. It starts when you wake up.
This is a description, not a challenge. Do not try it.
6 hours: nothing is wrong
A single long day is something the human body has always handled. Nothing here is a warning about one late night.
12 hours: the waste stays
One protein that builds up during the day is the same one found in the brains of people with Alzheimer's.
That needs saying carefully. One bad night does not cause dementia. Nobody has shown that. What has been shown is that the clearing happens in sleep. What decades of short sleep do is an open question.
18 hours: as slow as a drink-driver
You feel fine. Your reaction time now matches somebody at the legal drink-driving limit.
Alcohol tells you it is there. This does not. Your brain is already dropping brief moments in which nothing is processed.
24 hours: the hormones move
- The stress hormone stops falling in the evening as it should.
- The hormone that says "stop eating" goes quiet. The one that says "eat" gets louder, and it wants sugar.
- Testosterone in men has measurably dropped.
30 hours: the immune system
The cells that hunt infected cells are largely made and armed during sleep. After one missed night their number can fall sharply.
That is why people tend to fall ill after the deadline, not during it. Vaccines given to sleep-deprived people also produce a weaker response.
36 hours: the heart gets no break
Blood pressure is supposed to dip at night. With no sleep, the dip never comes. Inflammation markers rise.
One night does nothing permanent. It matters when it happens again and again.
48 hours: microsleeps
Your brain begins taking sleep without asking. For 3 to 15 seconds, parts of it switch off. Your eyes are open and nothing is received.
At 50 mph, a ten-second microsleep is 200 metres driven by nobody.
If lack of sleep ever hurts you, it will most likely be here. Tired driving causes crashes on a scale comparable to drink-driving. Cold air, loud music and an open window do not work. The only thing that works is stopping, and the only fix is sleep.
60 hours: sugar
Your body starts handling sugar like somebody with early diabetes. Your cells stop listening to insulin properly.
Healthy young people test as prediabetic after a few nights of short sleep, and it reverses when they sleep. That is the good news.
72 hours: perception comes apart
It begins at the edges. Things seem to move when you are not looking straight at them. Then come simple hallucinations: shapes, movement, somebody at the door who is not there.
This is not madness and it is not permanent. It is a brain building a picture from too little information. Emotional control has gone too, so small things become enormous.
You cannot judge your own state
Ask people how impaired they are, and after two days they say about the same as after one. Their performance keeps falling. Their estimate stops falling.
You lose the ability to judge how badly you are doing before you lose the ability to do it. So the fix has to be a rule, not a feeling.
You do not repay it in one night
On the first night back, the brain takes deep sleep first, because that is the cleaning. The dreaming stage is pushed to the following night and returns with interest.
That is why the vivid, tiring dreams come two nights later. Attention takes several ordinary nights to recover fully.
The short answer
Sleep is not rest. Lying on a sofa does none of this. Sleep is a maintenance window, and every system has booked time in it. The brain washes itself, the immune system arms, the heart stands down and the hormones reset.
